FORMS & FEEDBACK
Surveys.
Account-backed surveys with a familiar form editor, identified responses, and $5 one-time payments for each extra week.
WHAT IT IS
Ask something. Know who answered.
I built Surveys for feedback, team decisions, event plans, and questions that need a little structure. Use a familiar form editor with Questions, Responses, and Settings tabs, stacked question cards, and a preview before publishing. The interface follows Zentr’s black-and-purple visual system.
GET STARTED
Sign in to create
Continue with your Zentr account. Both creators and respondents must sign in. Responses are identified, not anonymous.
Build your form
Add a title, context, and 1–20 questions. Use multiple choice, checkboxes, short answers, paragraphs, or a linear scale from 1 to 5. Choice questions support 2–12 options. Duplicate, reorder, or remove questions, mark them required, and preview the form. New-form drafts are saved on the current device for the signed-in account.
Set a response window
Choose up to one week free from Settings. The clock starts when you publish. Editing a published form does not restart it.
Share and collect
Share the form link. Anyone with the link can read the questions, but submission requires a Zentr account. Sign-in returns respondents to the form. Each account may submit once.
Read responses
Responses offers Summary and Individual views. Summary shows choice counts, rating averages and text answers attributed to respondents. Individual shows each respondent’s Zentr account ID, name and email when supplied by Auth, with their answers and submission time. The owner-only CSV export includes these identity fields.
EXTRA TIME
The first week is free. Each extra week is $5.
Every new survey defaults to a free seven-day response window. You can choose a shorter window in Settings. From the published form’s Settings tab, choose 1–30 extra weeks and continue to Shopify. I use the existing Extend Expiration Date product at $5 USD per additional week. One purchased unit adds seven days (168 hours), two add fourteen days for $10, and so on. These are one-time payments, without a subscription or Zentr+ requirement. Shopify may add applicable taxes.
Checkout is tied to the account and survey that started it. The server verifies the current Shopify order, product, quantity, price, payment status and disputes before applying time. A browser return page cannot unlock an extension. Verification normally runs every five minutes; Already paid? Check payment requests a check. Network or store outages can delay updates.
Extra weeks follow the current response window. If already expired, the extension begins at the verified order’s payment time. Refunded, cancelled, test or disputed purchases do not provide active time. Rechecking a refund or dispute can shorten an extended window. Duplicate checks never apply the same purchase twice.
MANAGING YOUR FORM
Keep questions consistent as answers arrive.
Before the first response, you can edit a published form from Questions. After a response arrives, questions are locked to preserve answer meanings. Editing never changes the original expiry time.
Stop accepting responses closes the form immediately while preserving results. A manually closed survey cannot reopen through an extension. Delete form permanently removes the survey, responses and its app-side extension records; deletion does not issue a Shopify refund. Export responses first if needed.
Each creator can retain 100 surveys, create up to 25 per day, and collect up to 10,000 responses per survey. One account is not necessarily one person; the submission limit is enforced per Zentr account.
IDENTITY & PRIVACY
Your account is recorded with your response.
Before submission, the form states that the respondent’s Zentr account ID, name, and email when available will be recorded and shared with the survey creator. Identity comes from the verified signed-in account, never from an identity field supplied by the browser. The app labels unverified email addresses in individual results. Other respondents cannot view results.
Cloudflare serves the interface and Worker API. D1 stores survey definitions, answers, account identities, private sessions and purchase-verification records. Responses are stored on the server and are not end-to-end encrypted. New-form drafts are stored on the current device; published surveys and responses are stored on the server.
Zentr sign-in uses authorization code flow with S256 PKCE, an exact registered callback, verified ES256 tokens, and secure HttpOnly sessions. The server checks ownership, response identity, one-response-per-account limits and expiry. A dedicated authenticated bridge reuses the existing Shopify connection without copying store secrets. Shopify handles payment details. These providers may process ordinary request and connection metadata.
Closing does not delete results. Delete a form to remove its application records; provider backups and Shopify transaction records follow their own retention policies. Account deletion does not currently promise automatic deletion of survey records, so remove forms first when needed. Never enter passwords or payment details into a survey answer.
